To transfer the shares:

1. Complete the Stock Transfer Form.
2. The form must be signed by the shareholder(s) as the name appears on the certificate or account, guaranteed by a financial institution that participates in the Medallion Signature Guarantee Program. The shareholder may have to show proof of identification of former name, i.e., marriage license, divorce decree, etc.
3. Mail the completed form and stock certificates (if applicable) to:
