The manner in which stock is registered is governed by various state laws. Following are examples indicating the most common forms of stock registrations. If you have any questions regarding a specific registration, we suggest that you consult with an attorney.

Individual Registration

The legal name of the individual must be used. The individual's full first name should be used instead of initials, i.e. James R. Smith instead of J.R. Smith. Titles are not used. A married woman should use her first name, not that of her husband.

EXAMPLE: "JANE J. WILSON" NOT "MRS. JOHN W. WILSON"

Joint Registration

There are different types of tenancy (see below) used for the registration of securities in the names of multiple owners. The conjunction "or" is not used in registering joint stock ownership. Please specify the tenancy clause desired when transferring stock (if the tenancy clause is not specified, all items presented for transfer will be returned for more complete information).

Joint Tenants With Rights of Survivorship

When shares are held by two or more individuals as joint tenants with right of survivorship and one tenant dies, the entire interest belongs to the surviving tenant.

EXAMPLE: "JOHN S. DOE & JANE L. DOE JT TEN"

Tenants In Common

Form of ownership where each tenant owns an undivided interest. When one tenant dies, his interest passes to his estate.

EXAMPLE: "JOHN S. DOE & JANE L. DOE TEN COM"

Tenants By the Entireties

Recognized by certain states as an appropriate form of registration for two individuals who are married to each other. When one tenant dies the property passes to the survivor.

EXAMPLE: "JOHN S. DOE & JANE L. DOE TEN ENT"

Custodial Registration

The minor is the legal and beneficial owner and the custodian is merely the supervisor. The custodian collects, holds, manages, invests, and reinvests the custodial property for the benefit of the minor.

EXAMPLE: "JOHN R. SMITH (adult) CUST JUDY L. SMITH (minor) UNIF TRANS MIN ACT OH"

Trust Registration

For trust registrations, provide the name of the trustee(s), the name of the trust and/or beneficiary, and the date of the trust.

EXAMPLE: "JOHN R. SMITH TR U/A DTD 3/17/89 THE JOHN R. SMITH TRUST"

Transfer On Death (TOD) Registration

The owner of a security registered in TOD form must be a natural person or natural persons holding the security as an individual or joint owners with the right to survivorship (i.e. joint tenants or tenants by the entireties). TOD registrations by non-natural persons are not permitted since such owners have perpetual existence. Only one beneficiary per registration may be designated by the security owner.

Registration in TOD form shall be indicated by the abbreviation "TOD". The words "transfer on death" shall not be written out, nor shall the designation "POD", or the words "payable on death" be used.

The designation JT TEN (Joint Tenants) or TEN ENT (Tenants by the Entireties), if applicable, must be included in the registration. If a request for TOD registration in joint ownership does not specify the nature of the ownership, Shareholder Services Department will reject the transfer. The designations TEN COM (Tenants in Common) or a designation in any form of Community Property is not permitted since such tenancies lack the right of survivorship. Shares in the Shareholder Investment Program may also be registered in TOD form in compliance with this Rule. (TOD registrations shall be concluded by the phrase "SUBJECT TO TOD RULES".)

EXAMPLE: "JOHN S. SMITH TOD JANE R. SMITH SUBJECT TO TOD RULES"

EXAMPLE: "WILLIAM BROWN & ROBERT JONES JT TEN TOD JAMES WALKER SUBJECT TO TOD RULES"

EXAMPLE: "JOHN SMITH TOD MARY GREEN CUST ROBERT GREEN UNIF TRANS MIN ACT FL SUBJECT TO TOD RULES"

**Please note that the name of the beneficiary becomes part of the account title. We are not able to carry beneficiary information separate from the account title as is frequently done with brokerage accounts.

Corporate Registration

The corporate name should appear on the account without abbreviation.

EXAMPLE: "THE XYZ MANUFACTURING COMPANY"

Partnership Registration

It is recommended that investment clubs be organized as partnerships and the shares should be registered under a partnership registration.

EXAMPLE: "THE ABC INVESTMENT CLUB, A PARTNERSHIP"
