If you have lost a stock certificate, written notification from the shareholder requesting replacement of the securities is required. In your letter, please include the following information: 1. Name in which the certificate was registered, shareholder Social Security number and account number. 2. Number of shares on the certificate and certificate number, if you know it. 3. Reason for replacement. If the shareholder is deceased, the executor/administrator of the estate should contact Shareholder Services requesting replacement. We can accept a fax as written notification, however, an original, signed letter should be mailed to our office for inclusion in the replacement file. Email is also acceptable. Once Shareholder Services has received your request for certificate replacement, you will be mailed an affidavit for the lost certificate. You will need to complete the affidavit, have it notarized and then return it by mail to Shareholder Services. There is a charge of 2% of the value of the lost certificate. The minimum fee assessed is $10. Do not send the fee with your initial letter of notification. We will request the fee with the Affidavit of Loss.
