Educational Grant Request. P&G Pharmaceuticals collaborates with accredited continuing medical education providers to provide funding and assistance through educational grants.

Dear Grant Requestor,

This letter and the following Educational Grant Request (EGR) form are intended to provide information for a grant request in support of accredited or non-accredited continuing medical education (CME) to be considered by P&G Pharmaceuticals. The EGR form and appropriate documentation should be submitted by fax or via e-mail at least six weeks prior to the event. E-mail gigrants.im@pg.com or fax to

P&G Pharmaceuticals is pleased to collaborate with CME providers to provide funding and to satisfy other unsolicited requests for assistance from the provider as allowed by the FDA Guidance for SupportSM, the PhRMA Code on Interactions with Healthcare Professionals and the OIG Compliance Program Guidance for Pharmaceutical Manufacturers.

Specifically, P&G Pharmaceuticals will consider Educational Grant Requests to help meet medical educational needs in the following diseases and conditions only: digestive disease (focus on inflammatory bowel disease), osteoporosis and overactive bladder. Simply click on the appropriate link below to start the process.

Digestive Disease (IBD):	To save the document electronically, right-click and select Save Target As. Complete the entire application. Once completed, forward to gigrants.im@pg.com or fax to (513) 277-6579. EGR Form: http://www.pg.com/company/media/EGR.doc
Osteoporosis:	These will be handled by our partner in the Alliance for Better Bone Health, Sanofi-Aventis: www.sagrants.com.
Overactive Bladder:	These will be handled by our partner in this disease, Novartis: http://ngcs.novartis.com/index.jsp. You can call (888) NOW-NOVA for information.

For the digestive disease link, you will be directed to the EGR form. Please provide the completed EGR form, a cover letter containing the program description and amount requested (on letterhead), educational objectives, detailed budget and current-year W-9 form for committee review. Attach a copy of program content when available. Should you need more space than the EGR form allows, e.g., for budget itemization or description of enduring materials, please fax or e-mail the additional materials with the EGR. The CME contact listed on the EGR will be informed of the acceptance of the request within three weeks.
